you know palliative care setting we have a lot more scope to be able to be heavily involved in end-of-life care preferences so including getting documentation done like enduring powers of attorney for health like the advanced health directives as well as statement of choices the statement of choices is a form that is an updated version of an advance health directive and it's only really been in circulation for about the last five years so just to put things in perspective the advance health directive has been around for about 20 years and has an uptake in that in a community of around about 14 percent which is pretty poor so the statement of choices form is actually a much more interesting form and allows you to allocate your preferences and actually speaks to your values and beliefs as opposed to a medical model of saying if you have this do you want this and it's not an if-then scenario with tick boxes it's actually your chance and opportunity to write down some of the things that you would find unacceptable about your health care which i think is a really important point and often I call that the line in the sand question so at what point would you find any further treatment unacceptable or would you find something unacceptable in terms of your health care and the other thing that starts talking about is your preferences for where you would like to be cared for at your end-of-life so whether that's at home whether that's in hospital and a lot of those things can be changed but they're often things that aren't actually spoken about with families and don't actually have families don't have the opportunity to do that so the statement of choices gives you the scope to do that and I think it's a really important form
